This case happened in NSW a few years ago, and was caused by cooking middle eastern food on a hibachi style BBQ on a verandah. The briquettes were brought inside to provide a source of heat.

The whole story, which is not reported at the link, was pretty interesting, involving a migrant advocate/worker having an affair with a middle eastern client.

Charcoal has also been used as a method of suicide, most notably by Brad Delp, who was the singer in the rock band Boston. One of their hits was "More than a Feeling" (1976)
