I didn't think much of this at first, but the (handle?) seems to have purpose.

Very interesting. It looks like some kind of closed ended spanner. How big is it?
 
It is an old "Fountain" Handle.
For those who do not know what a fountain is,
Best described as a medium to large cast vessel that hung in a fire place to boil water. The spout that protruded out
had a tap on the end. They could be quite ornate as they were painted black and the spout was brass including the tap and handle.
 
Looks more like a handle off a piece of equipment, for opening or closing a valve of some sort. The square opening seems rather thick for a bottle opener, most are quite thin. Being so thick, I would have thought it was built for strength, hence belong to a more hefty purpose.
