I had problems with my shoulder and had ultra sound and x-ray, that were inconclusive. When I started to get pins and needles down my arm I had a MRI, and it showed a number of bulging disks and other issues in the upper vertebrae. It felt like shoulder pain, but was nothing to do with the shoulders, which were fine. After seeing the osteo surgeon, I stopped doing back squats, and introduced exercises like face pulls to strengthen up my neck and traps area. This worked, and I've had no pain or pins and needles since. Personally, even though it is expensive, I reckon an MRI is the best diagnostic tool - as it will show exactly what is going on. I'd also try some exercise (with the advice of a good trainer or a physio) before getting the injection.
